A study of the effectiveness of sildenafil citrate for erectile dysfunction (ED) in men in developing countries has found that a lower dose of the drug, Viagra (sildenafil citrate), was equally effective in controlling erectile dysfunction (ED) compared with placebo. The results were published in the (Journal of Sexual Medicine).
The study, reported in the (Journal of Sexual Medicine) included a total of 442 patients (mean age, 59.2 years) with ED. The patients were randomly assigned to receive one of the following treatments:
The mean time to onset of response was 11.5 hours in the sildenafil citrate group and 18.8 hours in the placebo group. The patients in the sildenafil citrate group had an average dose of 11.5 mg compared with 6.9 mg in the placebo group. The mean duration of improvement from the first sign of ED was 4.6 days in the sildenafil citrate group and 3.8 days in the placebo group. The average duration of sexual intercourse was 5.8 hours in the sildenafil citrate group and 4.5 hours in the placebo group.
The sildenafil citrate group also experienced a mean decrease of 4.2% in the total number of erections during intercourse compared with a mean decrease of 5.7% in the placebo group.
The sildenafil citrate group also reported a decrease in the number of erections during sexual activity (mean: 5.1, SD: 4.1) compared with a decrease of 3.7% in the placebo group.
The most common side-effects were headache, flushing, dyspepsia, nasal congestion, and nasal polyps. Sildenafil was well tolerated in the majority of the patients.
The overall incidence of adverse events was similar in the sildenafil citrate and placebo groups. However, the most common adverse event related to sildenafil was headache and the most common adverse event associated to sildenafil in the placebo group was nasal polyps. More than 50% of the patients discontinued treatment because of adverse events.
The most common adverse events were headache, nasal congestion, dyspepsia, flushing, nasal polyps, myalgia, back pain, and urinary retention.
About 40% of the patients in the sildenafil citrate group and 80% of the patients in the placebo group discontinued treatment because of adverse events.
The safety and efficacy of the treatment of ED is unknown. Patients with ED are at increased risk of sexual dysfunction and should be counseled about their potential risks and benefits. Patients with ED are at increased risk of other health problems, including diabetes and cardiovascular disease.

Men in the UK will be able to buy Viagra over the counter after consulting with a pharmacist and without a prescription for the first time today.
ViagraConnect manufacturer, Pfizer, estimates more than a fifth of men aged over 18 in Britain suffer from erectile dysfunction, but almost half of those over 40 have not sought medical help.
The erectile dysfunction drug will be available in 14,000 stores across Britain and via pharmacy websites.

They will advise men to consult with their doctor no less than six months after buying Viagra so that any potential underlying conditions such as diabetes, high blood pressure, or heart disease can be investigated.
A Boots spokesperson said that the man affected by erectile dysfunction should buy the drug themselves wherever practically possible.
Men with severe heart problems, those at high risk of heart problems, liver failure, kidney failure, strokes or those taking certain other medicines, will still need to be prescribed the drug under the supervision of a doctor.
A four-pack of the tablets will cost £19.99 and an eight-pack £34.99.
Men who buy it will be advised that they can take a 50 mg tablet an hour before having sex, but should not use more than one pill a day.
Viagra is is already available as an NHS prescription free of charge for those eligible not to pay for their prescriptions.
The pharmacist will decide how many packs will be supplied in a transaction.
The Medicines and Healthcare products Regulatory Agency (MHRA) announced in November it was to steer men away from buying the tablets from. In 2016 £17m worth of unlicensed and counterfeit Viagra was seized.
Kristie Sourial, Pfizer’s medical manager, said: “It’s important to remember that erectile dysfunction is a medical condition, and that the impact often goes beyond the physical symptoms – it can lead to men feeling anxious, depressed and lacking in confidence.
“Our hope is that the availability of Viagra Connect in pharmacies will encourage men who do nothing about their erectile dysfunction to seek help, and encourage those who are inclined to seek help to do so sooner.”
